08-09 Sports Physicals
All sports participants must have on file with the school a valid certificate of physical fitness issued no earlier than 13 months prior to the last event of any given sport season.
University Orthopaedics, 100 Butler Drive, Providence, RI, is holding a physical clinic on Saturday, July 26 from 11:00 a.m. to 1:30 p.m. – no appointment necessary. $10 fee. Student must be accompanied by parent or guardian, or you can get a release form at Feehan (office 8 a.m. to noon) if student will go alone. Directions to University Orthopaedics are also available at Feehan.
EXTRA-CURRICULA PARTICIPANTS:
If you joined the school accident insurance program, please remember that you must file any claim within 90 days of any iinjury suffered. Even if you have not received a bill for medical services or if you have a claim with another insurer, please file a claim with the school insurance ASAP. Claims should be reported to Miss Brandley Vice Principal for School Life. She will provide you with a claim form.
