Work Permit Procedure

Work Permits may be obtained from your town's Superintendent's Office. All children between the ages of 14 and 17 are required to obtain work permits before beginning employment (G.L. c. 149, §§ 86-89). A Work Permit Application must be completed before a permit can be issued.

Ages 14 and 15
 
Step 1: Student obtains a valid job offer from an employer and is instructed by the employer to obtain a work permit
 
Step 2: Student obtains a "Work Permit Application for 14 & 15 Year-Olds". Click here for the Massachusetts application.
 
Step 3: Pages 1 & 2 of the Work Permit Application are completed by the Employer, Physician and Parent/Guardian. Please note that students may not submit copies of health or immunization records.
 
Step 4: Return the completed pages to your Public High School's Superintendent's Office.
 
Step 5: Student is issued a work permit and presents completed work permit to the employer.
 
Ages 16 & 17
 
Step 1: Student obtains a valid job offer from an employer and is instructed by the employer to obtain a work permit.
 
Step 2: Student is issued a work permit and presents completed work permit to employer.
 
Note: Students must obtain their work permits in person from their Public High School's Superintendent's Office. They must sign the permit in the presence of the Superintendent's Office Staff Member.
